You must bring your doctor's order (prescription) on the day of your exam. Please, be aware that we can't do the procedure without this order. We accept medical prescriptions from other States of the union, Canada, Europe, some Latin American and Caribbean countries. Your provider can use also,  our own prescription. Just fill out the form and select the desired exam  (See/download our form on the homepage).
